Place frozen dish in fridge to thaw overnight.
Preheat oven to 375°F.
Remove lid, cover container with foil, and bake for 40 minutes.
Remove the foil and continue to cook an additional 10-20 minutes until the cheese is bubbly and lightly golden brown, and the internal temperature is 165 degrees.
Sprinkle with freshly grated parmesan before serving.
